It is Follicular Unit Extraction or simple Follicular Transfer. In this hair follicles individually har-vested and then transplanted to the recipient site. The survival of follicular units is crucial and is the key element in deciding the success of hair transplant. It is minimally invasive and there is no need of stitches.
Hair fall in men is highly sensitive to DHT(dihydrotestosterone). Follicular units, called grafts, are extracted from DHT resistant areas of the scalp. These areas include the back and sides of the head. These areas are known as the donor sites and the follicles are extracted using a multi-step process. A small punch ranging from 0.6 to 0.8 mm is used to penetrate into the soft tissues around a follicular unit. The direction of the punch varies from follicle to follicle and should be in the direction of the hair on the surface. A dull punch is the safest method of punching that separates the follicle from the surrounding soft tissues. This follicle is then extracted using a forceps and is ready to be implanted. Due to the extremely small hole created at donor sites, there is no scarring and no visible wound.
Once the hair follicular units are extracted they need either immediate placement into the recipient site or need to be preserved till the end of harvesting procedure. Each follicular unit contains one to four hair follicles. Along with the hair follicle, surrounding soft tissues including epidermis and dermis are also extracted to provide a complete follicular system. Due to large number of graft wastage i.e. graft transection; a higher number of hair follicles are extracted, compared to what's required. To prevent graft wastage, harvested follicular units that are not transplanted immediately, are placed into a holding solution, which is typically a saline solution. Alternative holding solutions like buffers, nutrient mediums containing antioxidants or Platelet rich plasma solutions containing growth factors may also be used. These solutions prevent desiccation and dehydration of follicular units.
Each follicular unit contains one to four hair follicles. Once the follicular units are harvested, the recipient or the balding sites are prepared for implantation. The physician makes small incisions at the recipient site in accordance with the patient's natural hair growth pattern. The follicular units are then placed into the tiny recipient sites using a forceps and other similar tools. Often advanced tools like the Choi implanter pen are being used nowadays. The follicular units may be placed into pre made recipient sites, as soon as they are made, or after the complete harvesting process is complete. The former technique of immediately placing grafts, after they are made, is known as "stick and place" technique. The follicular units require strategic placement to give the best and most natural looking results.
